Part 2 of my Aug 2016 conversation with Dr. Kraft. Father of the Insulin Assay, and one of the most under-recognised greats of medicine and science. Here at aged 95 he talks of old times, relating experiences from the early days of antibiotics. We also discuss the methods used to manage TB back then,  including pretty successful altitude treatments. Some extraordinary cases are discussed, including a young man who survived the Death March of Bataan harbouring massive disease. We also visit the unusual autopsy findings for president Franklin D Roosevelt's wife, Eleanor (who died at the young age of 40).

Ireland's Top Dietician: Sugar is not a big deal, and #LCHF is Stupid Fad ;-)

The title says it all - one of Ireland's top consultant dieticians (with multiple nutrition degrees) let's us know what healthy eating is all about. It's only about the calories - that's what counts. Breakfast cereals are full of great nutrition, with relatively few calories. Carbs are irrelevant physiologically - it's the calories stupid. We're just eating too much is all. Most importantly, we need to properly follow the current guidelines and then "no-one would be overweight or obese".

Here's the "scientific" study referenced - even worse than the usual associational claptrap http://gibneyonfood.blogspot.nl/2017/01/think-sweet-stay-slim.html Excerpt: "The NutriSanté study is an internet-based study, initiated in 2009.....Each month, volunteers are invited to complete questionnaires related among other things to food behaviour.....". Yep - an internet-survey based associational twaddle asking about food preferences. Eight out of ten scientist cats in our survey said this paper is junk (the other two are on food company payroll).

TED Talk from Dr. Rangan Chatterjee: putting Insulin Resistance centre-stage!

Too busy for posts lately, but can't miss this one. I first met Rangan in London last November at Darryl Edward's "Health Unplugged" conference. He appeared out of nowhere, treating me to an enormous handshake - and also his trademark intense authenticity. This guy means everything he says - and he's bang on the money to boot.

Hope you enjoy this excellent TED talk (his first) as much as I did. It's so fulfilling to see the focus moving to genuine root cause resolution. Every doctor needs to view the world as Rangan does; attend to the causes - not the symptoms...! :-)

"The Complete Guide to Fasting" (Dr. Jason Fung / Jimmy Moore) - Book Review !

I have long practised the art of fasting; for the weight control benefits sure – but also for the health and mental wellbeing that it affords. On the latter it is much more than mere wellbeing by the way. Fasting enables an extraordinary boost in mental acuity. With no doubt whatsoever, I am at my sharpest and most capable when fasted for about 20 hours or more. Thus I prepare for stressful or challenging situations – by not eating for a long period in advance.

Reading “The Complete Guide to Fasting” was a hugely enjoyable experience. While doing so the above quote from Julius Caesar sprang into my mind. Caesar sees the danger in Cassius, his intellect honed from lack of overfeeding. Deep down might we all like to be Cassius?  Perhaps not the dangerous piece. But certainly the lean and sharp-minded aspects. How do we achieve this state of body and mind? How do we reverse metabolic disease and poor health status? Is it just by old-fashioned calorie-counting, and perhaps getting your macros right (i.e. healthy LCHF)?

I don’t think so. I rather agree with Jason and Jimmy’s premise in this book. The ‘diet-and-exercise’ and ‘eat at regular intervals’ advice has been trotted out for the past 50 years. But the dietary elements (and eating patterns encouraged) flew in the face of evolutionary evidence. With high carbohydrate and regular ‘grainy snacks’, we have spawned an enormous diabetes epidemic (over half of adult Americans are now pre-diabetic or diabetic). The modern population is indeed in a difficult place. It will require a return to evolutionary principles to set us free.

The book lays out all aspects of Fasting’s ancient practice, explaining the science behind the many benefits. It starts off by contrasting the terms ‘starvation’ and ‘fasting’. The former is forced on a person, the latter is by choice. There is an enormous difference. You will however hear people deceitfully use the term ‘starvation’ in order to attack the practice of fasting. Ignore these idiots. We see that fasting essentially switches the fuel source from what you put in your mouth, to what you carry around your middle. The biochemistry is beautiful – elegantly switching to ancient processes of bodyfat oxidation, both healthy and entirely appropriate. Many other biochemical benefits are triggered by fasting, including the positive boost in Human Growth Hormone (HGH). This has a key role in preserving muscle and leads to many other parallel benefits for longevity.

Chapter two covers the fascinating historical background of the practice itself. All religions have fasted in various ways. Nearly all cultures have valued its benefits. Over the past few decades, however, our western industrialised society has consistently undermined the ancient practice. Why is this? Well, the food, advertising and other industries have a huge stake in the consumer society. They care little of the threat from ‘reduce your calories’ (they are well aware that this jaded junk-advice doesn’t work for long). The concept of deploying ancestral wisdom is another matter entirely. To have fasting become a valued and common pursuit…is quite a frightening spectre to the money men. And so it must be rubbished and dismissed as ‘unhealthy and dangerous’.  In chapter three we get an entertaining run-through of all the myths that have been crafted to protect the profits. We also get a super summary of the truth that belies every single myth. I was reminded of the cholesterol myths we have suffered for so long – the same ruinous rulebook has been applied to fasting. We have been made fools of for decades – but now it is time to wake the hell up.

Chapters four brings us on a journey that is as convincing as it is captivating. Have you any idea of the sheer diversity and magnitude of fasting’s advantages? You certainly will after these chapters – and want to start getting hold of them for yourself! What emerges is a picture in stark contrast to what we’ve been told for generations. With fasting the bad gets minimised, the good gets better – across a dizzying array of advantages you can create for yourself. All are lined up like in an ammo belt: Simple, Free, Convenient, Pleasure-Enabling, Powerful, Flexible, …and it works with ANY chosen diet. Explained too is fasting as one of the most powerful tools in the box; it can deal with challenges that are resistant to LCHF and many other approaches – challenges which quail in the face of fasting. It is that powerful. You are free to wield this tool for yourself…after internalising the content of this book.

The next chapters bring us through the profound problems of weight-loss and type 2 diabetes. You guessed it – the power of fasting comes into its own here. Peppered throughout the text are real-life affidavits from Jason’s practice which illustrate what can be achieved. As always, the supporting science is referred to and explained – without slowing the pace with heavy or boring detail. We get to see the enormous benefits which fasting can deliver against Alzheimer’s, Heart Disease and many other modern inflammatory conditions. As mentioned I have been fasting for many years now, and have strong thoughts on the ways in which hunger can be mastered with the right approaches; in chapter nine we get a superb summary of how to do this. Importantly, there are explanations of the mechanisms behind detrimental feelings of hunger – and how they can be sidestepped gracefully. I love it.

Time is pressing so here I will wrap up. There is a comprehensive Section 2 which gives all the protocols for successfully implementing a range of fasting regimes. All common variants are covered carefully, to enable all types of fasting. With this section you can become an expert in the art. You’ll never look back. The final section 3 has a range of delicious recipes, for the times when you are not indulging in the mind and body-enhancing attributes of the ancient remedy.

Get this book, and you too can be a kind of Cassius; lean, focussed and sharp-witted. But without the dangerous streak maybe…unless you like that sort of thing !

The mice now speak: Insulin Resistance freed from the confounding of 'cholesterol'

Quick post on some interesting new research sent on to me by Raymund Edwards (thanks Raymund!). Only speed-read it but good to get it out for review. The old cholesterol and atherosclerosis thing has been doing the rounds for decades, while the data points to insulin-related dysfunction being vastly more important. Here we have a team who are helping to remove the 'cholesterol noise' from the insulin picture. I'll simply reproduce Raymund's facebook post in full below. Bottom line is that they've produced a mouse model which separates the 'cholesterol confusion' from the 'insulin resistance imperative'. They have in short demonstrated atherosclerotic acceleration through insulin-mediated action - WITHOUT the cholesterol confounding. Cholesterol metrics gone awry are most often a by-product of insulin/glucose dysfunction, and the 'cholesterol' gets mistaken for cause. So kudos to these researchers - if we had work like this a few decades ago, we might not be suffering the diabetic farce that we are in today...

--> Individuals with metabolic diseases are at a higher risk of developing atherosclerosis, a leading cause of death in the United States and worldwide.

Earlier studies have linked dyslipidemia to the initiation and progression of atherosclerosis. However, recent clinical studies raised concerns about the efficacy of lowering plasma cholesterol levels in the progression of atherosclerosis. Although insulin resistance is associated with increased incidence of cardiovascular disease, whether it leads to atherosclerosis independently of its accompanying dyslipidemia remains unclear, largely because of the lack of a suitable animal model to address this question.

The CarcinoEmbryonic Antigen-related Cell Adhesion Molecule-1 (CEACAM1) regulates insulin sensitivity by promoting insulin clearance in liver. Accordingly, global null deletion of Ceacam1 gene impairs hepatic insulin clearance and causes hyperinsulinemia, which in turn, results in systemic insulin resistance.

Preliminary data show: (i) that global Cc1–/– null mice develop early atherosclerotic lesions and vascular dysfunction even under normal feeding conditions, and (ii) that this occurs in the absence of hyperlipidemia, despite VLDL/LDL cholesterol levels that are usually associated with atherosclerosis regression, not development.

This unique animal model of atherogenesis with isolated insulin resistance in the absence of hyperlipidemia demonstrates that systemic insulin resistance resulting from hyperinsulinemia leads to vascular dysfunction and atherosclerosis in the absence of hyperlipidemia.

Because phosphorylation of CEACAM1 by both insulin and VEGF receptors regulates Akt1 activation of endothelial Nitric Oxide Synthase (eNOS), an essential step in mediating endothelial function, it is reasonable to propose that CEACAM1 is the shared downstream element in VEGF and insulin signaling in endothelial cells, whose inactivation impinges upon both pathways and causes endothelial dysfunction in insulin resistance.

We and others, have shown that mice with global deletion of Ceacam1 gene (Cc1–/–) exhibit hyperinsulinemia, resulting from impaired hepatic insulin clearance. Hyperinsulinemia in these mice leads to insulin resistance, increased liver triglyceride content and de novo lipogenesis, which drive VLDL-ApoB secretion and triglyceride accumulation in adipose tissue, causing visceral obesity and raising serum fatty acid levels. Thus, the Cc1–/– mouse is a
model of hyperinsulinemia causing insulin resistance.

•• Preliminary data show: (i) that Cc1–/– mice with global Ceacam1 deletion spontaneously develop early atherosclerotic lesions with impaired endothelial-dependent vasodilation under normal feeding conditions, and (ii) that this occurs despite VLDL/LDL cholesterol levels that are usually associated with atherosclerosis regression, not initiation.

Therefore, Ceacam1 deficiency causes hyperinsulinemia and insulin resistance, and promotes lipid-independent vascular toxicity. This makes Cc1–/– a unique animal model of spontaneous
atherosclerosis with isolated insulin resistance in the absence of hyperlipidemia, and provides an in vivo demonstration of distinct CEACAM1-dependent mechanisms linking insulin resistance to atherosclerosis.

Native Human Cholesterol/LDL as a significant causal driver of Atherosclerosis? Never made sense to me to be honest. Data doesn't add up at all. #LDL Cholesterol - A Nastily Misleading 'Risk Factor' #LCHF

Well here we go again - deja vu. Just looking up some stuff on Free Fatty Acids this evening and came across this paper. These guys made the old mistake of measuring LDL in (a) healthy people, (b) Diabetics and (c) Acute Myocardial Infarction victims. Note that for all groups, patients on lipid-lowering drugs were excluded (i.e. this is a proper study, not confounded with meds). And so we get a glimpse of the fat, sweaty Cholesterol Emperor again. Such careless researchers, to let the cat out of the bag like this! Have a look at what did matter between the different types of people (with LDL at top middle, irrelevant as usual):

And below in tabular form - Red boxes show relevant stuff, while the dashed box shows the junk-science distraction-factor....of the industry darling, LDLc:

But did they call out the LDL irrelevancy in their conclusion, and ask some salient scientific questions? No, of course they didn't. Because you're not allowed to question the LDL dogma, or it's bye-bye to your future funding. That's why we've had to endure the LDLc distraction for decades. And these guys did worse than ignore the obvious LDL question in their conclusion piece. Just look at their actual comments on LDL - what a deceitful joke:
